What Is the Facegenics Midface Expander?

The Facegenics Midface Expander (FME), sometimes referred to as a Facial Midline Expander, is a specialized orthodontic appliance that uses Temporary Anchorage Devices (TADs) to create expansion of the upper jaw.

TADs are small titanium anchors that are temporarily placed within the palate and serve as stable support points for the expander. Because the appliance is anchored directly to the bone rather than the teeth, expansion forces are directed toward the skeletal structures of the face and jaw.

This approach allows for more efficient and predictable expansion while minimizing unwanted tooth movement.

How Does FME Work?

Traditional palatal expanders primarily push against the teeth, which can sometimes result in dental tipping rather than true skeletal expansion.

The FME uses approximately 6 to 12 Temporary Anchorage Devices (TADs) that are anchored directly into the palatal bone. These anchors provide a stable foundation that allows the appliance to apply forces directly to the upper jaw.

As the appliance is activated over time, the maxilla gradually widens, creating additional space within the upper jaw and surrounding structures.

Because the appliance is bone-borne, it is often referred to as a "pure bone-borne expander."

Benefits of FME Treatment

True Skeletal Expansion

By working directly with the bone rather than relying primarily on the teeth, FME is designed to promote expansion of the upper jaw itself, helping address underlying skeletal deficiencies.

Improved Airway Development

Expanding the upper jaw can increase the volume of the nasal cavity, which may support improved airflow through the nose.

For some patients, airway-focused orthodontic treatment may be considered as part of a broader treatment approach for concerns related to sleep-disordered breathing.

Enhanced Breathing Function

Many patients seek expansion treatment because of concerns regarding nasal breathing. Increasing nasal cavity volume may contribute to improved nasal airflow and overall breathing comfort.

Wider, More Balanced Smile

A narrow upper jaw can contribute to a constricted smile appearance. FME treatment may help create a broader smile arc and improved smile aesthetics.

Adult-Friendly Alternative

Historically, significant maxillary expansion in adults often required surgical intervention. FME offers a non-surgical approach that may allow qualified adult patients to achieve skeletal expansion without orthognathic surgery.

Who May Benefit From FME?

FME may be considered for adults experiencing:

  • Narrow upper jaws (maxillary constriction)
  • Crowded teeth related to jaw width limitations
  • Constricted smile appearance
  • Airway concerns
  • Difficulty with nasal breathing
  • Interest in non-surgical skeletal expansion
  • Orthodontic treatment requiring additional arch development

A comprehensive orthodontic examination, including appropriate diagnostic records and imaging, is necessary to determine whether FME or another treatment approach may be appropriate.

FME vs. Traditional Palatal Expanders

Traditional Expanders

What to Expect During Treatment

Treatment begins with a comprehensive orthodontic examination and diagnostic records. If FME is determined to be an appropriate option, Temporary Anchorage Devices are placed within the palate and connected to the expander appliance.

The appliance is gradually activated according to your orthodontist's instructions, allowing controlled expansion of the upper jaw over time. Throughout treatment, periodic visits are scheduled to monitor progress and ensure optimal outcomes.
